By turning onto the back or side position during sleep, a big height difference is created in the neck area, if one assumes the optimal head position is kept.
When this height difference during turning in your sleep is not taken into account and the optimal position cannot be kept, blood vessels, nerves and lymph glands can be closed off under pressure.
Pillow height difference (overview)
Gravity and body weight presses on blood vessels, nerve strands and lymph glands in every sleeping position. That results in restrictions to the blood circulation, stiffness of the neck and pain during the night. In order to avoid this unpleasantness during a long sleep, every person unconsciously turns up to 30 times (different from person to person).
To ensure a good sleep in the side position, one needs a higher pillow for the spine to stay straight. However, if the same pillow is used in the back position, the trapezius muscles and the muscle groups around the neck are tensioned. During sleeping pressure is exerted on the muscles, blood vessels, nerve strands and lymph glands in the underside of body through the force of gravity and body weight. During a longer period in the same sleep position one then soon becomes aware of the uneven blood circulation and the irritation of the nerves. Therefore one unconsciously turns during sleep. Up till now there was no single pillow that automatically provides the ideal height for the head position in both the back and side positions. For the back position a low pillow is the ideal and for the side position a higher pillow.

Man has developed over millions of years and is now walking upright. Because of this skeleton construction and the muscles man needs a pillow for sleeping. The neck must carry a weight of 4-5kg. The head houses important functional strands for the eyes, nose, tongue and ears. The head rests on the neck, which must move it horizontally and vertically at will.
The spine has 7 cervical vertebrae on top of one another. Compared to the head and chests the neck is quite narrow for this responsibility. The vertebral canal runs inside the length of the spinal column, through which the nerve strands runs in the spinal marrow. To protect these sensitive strands and to move the spine freely as an axis, several muscle groups cross each other, which join the head to the body and stabilise it.
In a standing position the spine, consisting of 7 cervical vertebrae, 12 thoracic vertebrae, 5 lumbar vertebrae, 5 sacral vertebrae, 3-6 coccygeal vertebrae, in other words a total of 32 - 36 vertebrae, appears straight from the top to the bottom in the front and rear view. The profile view however appears as an S curve. Because of this S design it can carry the head and function as a spring against gravitational forces and oscillations.
Due to the S curve construction the spine is bent slightly forward. For that reason the head lies slightly in front of the line, which runs from the buttocks upwards through the back. During standing and sleeping this S line is maintained, enabling every muscle to hold the heavy head with little exertion. This way muscle fatigue is relatively low, blood circulation is unrestricted and the nerves are not put under pressure. Therefore one needs a low pillow in the back position for a good and healthy sleep.

Sleeping in the back position is often the most comfortable. However, after a longer time in this position the body weight and the force of gravity exert pressure on the blood vessels, which leads to nerve tension in the underside of the body. That causes a blockage of the blood circulation, preventing the transporting of enough oxygen to the muscles. Lactic acid accumulates in the muscles, leading to fatigue and pain. Pain is also caused by pressure on the nerves. That leads to repeated turning during sleep.
When the sleep position is changed, an ordinary pillow does not adjust to the height of the head, which means that the head lies either too high or too low. To compensate for that one intuitively puts your hand under the pillow or try to reshape the pillow. That disrupts healthy deep sleep.
On-the-side sleepers and people convinced of remaining in the same position the whole night often use a higher pillow. When one turns onto the back, extreme pressure is exerted on the nerve strands and on the trapezius and levator scapula muscles, which disrupts sleep. In this position many people unconsciously remove the pillow and continue sleeping without it. Due to the years of habit they often put it back when turning to the side position.
Both sleep positions are very different, because in the side position the shoulders are pulled forward. In order to overcome the height difference between the outer shoulder line and line of the ear, a higher pillow is needed. A pillow for the back position is not suitable. Mass-produced pillows also rarely provide the desired effect, because the measurement is taken in the back position. That again causes the mentioned difference in the side position. The same applies in side position pillows.
